Insurance and Payment Information
We require that services be paid in full on the day received, unless other arrangements are made ahead of time. Mercy Surgical Services works closely with several insurance carriers, so we can bill these carriers for you. However, any co-pay, deductible, or co-insurance amount must be paid on the date of service. Arrangements can be made for special circumstances. You will be expected to present your insurance card at each visit. Please direct any questions about your coverage or billing statement to (989) 348-1040 or 1-888-999-6011 (toll free).

Financial Policy
Insurance Philosophy: Payment
is due at the time of service. If you have an insurance policy you need to understand that this is a contract between you and your insurance company. As a service to you, we will file your insurance claim to your insurance for medical services. If your insurance company does not pay the balance within a reasonable period, we will look to you for payment.

Preauthorization: Your insurance company may require prior authorization for services. You will need to contact our office to make sure we have an authorization before services are rendered. If no prior authorization has been obtained, you will be required to pay at time of service.

Non-Covered Services: Not all services performed or ordered by your provider may be covered by your insurance company. In the event your insurance plan determines a service to be “not covered,” you will be responsible for the complete charge.

Copays and Deductibles: Any required copays and/or deductibles are your responsibility. These will be required to be paid at the time of your visit.

Credit Cards: For your convenience, we accept credit card payment, including: Master Card, Visa, American Express, and Discover.

Prompt Payment discount: We provide a “prompt pay” discount for all patients and insurance companies who pay at time of service for charges of $500 or greater (excludes cosmetic procedures).

Collection: Mercy Surgical Services (MSS) uses a collection agency for balances that are not paid and have been identified as your responsibility.

NSF Checks: Any non-sufficient fund (NSF) checks written to MSS will be charged an additional $30 to cover our bank charges and handling fees.

Payment Plan/Financial Assistance: Our billing department will help you set up a monthly payment plan if needed. Also, we will assist you in determining if you could be eligible to receive financial assistance from MSS.

It is the patient’s responsibility to know what his/her insurance covers.
